This module will focus on care navigation to assist the aging population, including end of life issues and rights in a residential care facility.
COMPETENCIES
Upon completion of the unit the student will be able to:

- Apply knowledge of care coordination to facilitate individual plan of care outcomes
- Use communication methods to build professional relationships and positive interactions across multiple cultures and demographics
