Lesson 5 Navigation as a System Approach

Objectives:

  1. Define evidence-based practice and describe how these practices are utilized in the behavioral health services and the healthcare system.
  2. Apply soft skills and communication strategies to engage interdisciplinary, primary care, and behavioral health teams members in the coordination of intervention and care.
  3. List three evidence-based practice strategies for working in an interdisciplinary team to perform behavioral health navigation and coordination of services.
  4. Describe the motivational interviewing process and how it can be applied in care navigation.
  5. Describe the SBIRT and GAIN-SS process and how it can be applied in care navigation.
  6. Explain how screening tools are used in integrated teams.

Course Outcomes met in this Lesson

  • Define behavioral health navigation for working with individuals and families.
  • Describe current systems and referral processes of behavioral health services (e.g., mental health, chemical dependency, domestic violence, suicide, and trauma).
  • Engage interdisciplinary, primary care, and behavioral health teams members in the coordination of intervention and care.
