This module will focus on the emotional, social, and psychological impacts of behavioral health issues in the primary care setting and how care navigation can benefit the client in crisis.
Upon completion of the unit the student will be able to:
- Navigate the healthcare and human services systems to connect client to resources
- Use knowledge of issues, warning signs, and risks that may arise in dealing with behavioral health conditions and seek resources to support care
- Apply knowledge of care coordination to facilitate individual plan of care outcomes
